The Grand at Twin Lakes is hosting a CE Program breakfast on August 23rd for Voice Your Choice & POLST presented by Rachael Talleen from JourneyCare. 8:30 a.m. Registration and Breakfast. 9:00 a.m. - 10:00 a.m. CE Program Presentation. Participants will learn about the advance care planning options as well as how to complete the PLOST form. As a health care professional, you may have the critical role of educating patients about the benefits of ensuring that their healthcare wishes are respected if they are no longer able to speak for themselves.
